Take the time to think about what your wishes are and how prepared you are for the end of life. We can look at a variety of things, from practical preparations to planning your own funeral. It may take away some of the fear, and give you peace of mind. Help ensure that when you die it is as easy as possible for your loved ones. I can work with individuals and small groups. My style is relaxed and conversational. Lets talk, and work things out together.

Advance care planning can include some or all of these areas:

Your will - is it up to date?
 

Establishing a DNACPR order (Do Not Resuscitate) if you want one.

 

Tying up your loose ends / death cleaning / housekeeping and practical matters for the various areas of your life.

Setting up Lasting Powers of Attorney for Health & Welfare, and/or Property and Finance.
 

Gaining an understanding of what 'normal dying' looks like.

 

Funeral planning - talking through what you do and don't want for your funeral.

Setting up an Advance Decision to Refuse Treatment with a supporting Advance Statement.
 

Talking through your last wishes and what might feel important to you.
